Interested Observer, thank you for getting this information for us. I agree that it doesn't look like good news at all.
At first I was positive about BioAmide's prospectes but over the past few weeks it occurred to me that they might still be in the realm of pure theory. I believe they are.
The basic premise of this idea -- implanting dermal papilla cells into the scalp in some kind of matrix, to keep them together -- is something I first saw proposed here on HairSite. Some poster (I forgot who) proposed the idea well over a year ago. I can't remember exactly when the posts were made, or the nickname of the poster, but it was definitely proposed by this person several times. It has even occurred to me that this poster might have been Tom Barrows. I don't know much about Dr. Barrows, or if he's experiencing hairloss. But I think this is for all practical purposes such a wild and innovative idea, that I find it hard to believe that someone would post it on HairSite, and then a scientist would attempt it independently. I think it's possible that Dr. Barrows might view HairSite regularly because of an obvious interest in hairloss and hair regeneration.
In any event, with respect to the information you posted, I am interested as to why they implanted the cells behind the ears of these patients. I assume this was because if anything strange happened, or if there was an unsightly reaction, it wouldn't be at the front of the scalp where it would be highly visible.
Also, where they inserted scaffolds with clumped cells, they made what seem to be rather large incisions, and they didn't want to expose their patients to visible scarring, especially if no hair grew.
One slight problem (as I see it) is that the area behind the ears is NOT a place where MPB normally occurs. Therefore it is not an area where you would expect to find a lot of DHT-susceptible miniaturized follicles in the average person with MPB.
If we assume that Gho's and Unger's HM/CT work is based on the injected cells inducing miniaturized follicles to regenerate into large, terminal follicles, then if cells are injected behind the ears, there may be no miniaturized follicles to be induced. Thus, in BioAmide's Group A, it makes sense that no hair grew, because there were no miniaturized follicles to induce.
In BioAmide Group B, one out of four patients grew hair, when scaffolding with clumped DP cells were injected. This (in my opinion) would suggest the de novo creation of brand new follicles.
Therefore it's possible that clumping the cells in the scaffolding allows the creation of some type of "pre-follicular mass" which in SOME instances will develop into a viable hair follicle growing hair. But this may only happen where conditions are PERFECT (adequate numbers of cells, cells remaining clumped together, perhaps an adequate blood supply, etc.) Therefore in Group B, only 1/4 patients grew any hair.
I believe that the reason BioAmide made what seems like large incisions using scalpels was not something inherently necessary with the procedure; I believe they CAN use much smaller (Woods like) incisions to do the same thing -- insert bits of scaffold with cells -- but didn't do this simply because of lack of experience in that area, and the desire to get the tests underway as soon as possible to see if something would grow.
Basically this phase of their research was just "proof of concept" -- it didn't matter too much how the scaffolds were implanted, they could modify that process later and make it less invasive.
But all in all, I agree with Interested Observer and am NOT positive about this particular research. Maybe BioAmide now has new research which has shown better results, or maybe now they are growing the follicles or "pre-follicles" in vitro, to implant in the scalp later on. I think this is probably the only way to go for BioAmide. I think their particular approach DOES NOT lend itself to trying to grow brand new follicles in the scalp.
I think when BioAmide first undertook this research, they had no idea whether previous cell therapy experiments (Gho, Unger, Vogel, Jahoda) were creating brand new follicles in the skin or simply inducing dormant miniaturized follicles to grow larger.
I think Barrows may have assumed the other CT experiments worked by growing brand new follicles. Thus, he thought you could simply inject or implant DP cells into the skin anywhere, and as long as they were kept together somehow in clumps, they'd grow a new follicle. (Remember, Dr. Barrows is an organic materials scientist, a biomaterials scientist who specializes in creating bioabsorbable polymers for tissue engineering. He is NOT a dermatologist like Dr. Gho or a skin scientist like Dr. Sun!)
In most instances, what Barrows is doing will probably NOT work. But I think it's really not that simple. Gho, Unger, and yes, Jahoda (see www.hairlosshelp.com, front page) are not creating new follicles, they are regenerating dormant follicles. On the other hand, Barrows approach is really to create NEW follicles by clumping cells together to form a mass which will grow into a brand new follicle. The problem is that this probably won't work well if done in vivo, in the patient's skin. You have to grow the entire follicle in vitro, then implant it into the skin.
